Program Registration Payment Terms:

Summer Programs: HST is not included in the price and will be applied to all camps. Deposit of $100 is required which is non-refundable after April 15, 2017. Full payment is due on and non-refundable after June 1, 2017. If you elect to pay with cheque, money order, or cash the spot will be held for two weeks until deposit or full payment is received.

Atlantic Hockey Showcase / Atlantic Female Prospect Camp / Advanced Prep Camp: HST is not included in the price and will be applied to all camps. Deposit of $100 is required which is non-refundable after March 15, 2017. Full payment is due on and non-refundable after May 1, 2017. Note: If you elect to pay with cheque, money order, or cash the spot will be held for two weeks until deposit or full payment is received.

Participant Waiver: In consideration of the participant and his/her parent being permitted to register the participants to participate in the ANDREWS HOCKEY GROWTH PROGRAMS I, the undersigned parent/guardian hereby release and discharge ANDREWS HOCKEY GROWTH PROGRAMS from any and all claims, demands, actions and causes of action which I may have for any damages, loss or injury suffered by my child or incurred by me and resulting directly or indirectly from the participation of such child in such program. I hereby undertake to indemnify ANDREWS HOCKEY GROWTH PROGRAMS and its servants, agents and employees and hold them harmless from and in respect of any and all claims, demands, actions and proceedings which may be brought by or on behalf of my said child against ANDREWS HOCKEY GROWTH PROGRAMS arising out of his participation in the aforesaid program and in respect of any damages, loss or injury incurred by him during or as a result of any damages, loss or injury incurred by him suing or as a result of such participation including all costs and expenses incurred in defending any and all claims, demands, actions and proceedings.

Photo/Video Release: I understand that ANDREWS HOCKEY GROWTH PROGRAMS may photograph or take video footage of my child, in relation to my child’s participation in an instructional hockey camp. I hereby give permission to ANDREWS HOCKEY GROWTH PROGRAMS, its affiliates and assigns, to publish in print, electronic or video format the likeness of image of my child, including but not limited to publishing the likeness or image of my child on the internet on such sites as deemed appropriate by ANDREWS HOCKEY GROWTH PROGRAMS. I hereby release ANDREWS HOCKEY GROWTH PROGRAMS, its affiliates and assigns from all claims to proprietary rights including copyright protection in relation to the use of the photographic and video materials described herein, and I also hereby release ANDREWS HOCKEY GROWTH PROGRAMS, its affiliates and assigns from any claims whatsoever arising from the publishing of my child’s likeness or image in print, electronic or video format.

Pool Waiver: In consideration of the participant(s) being accepted and allowed to participate in the aquatic swim program as a component of ANDREWS HOCKEY GROWTH PROGRAMS, I hereby waive on behalf of myself and said participant(s) against ANDREWS HOCKEY GROWTH PROGRAMS, CARI Complex, their agents, employees or other persons acting on their behalf, all claims, demands, damages, suits, or other proceedings attributable to any such accident arising in any manner out of this program resulting in death, injury, or damage to either persons or property. Please note: There is some risk associated with this activity.

Spring Hockey Registration Payment Terms: HST is not included in the price and will be applied to all camps. For all Spring Hockey Registrations, a deposit of $200 is required which is non-refundable. Full payment is due on March 1, 2017 and all payments made are non-refundable after Jan 1, 2017. Note: By signing the payment authorization form, you authorize Andrews Hockey Growth Programs Inc to charge your card on the dates and for the corresponding amounts listed. If participants registration forms are not received by Oct 15, 2016 Andrews Hockey Growth Programs is not obligated to hold a spot for that participant.

Participant Waiver: In consideration of the participant and his/her parent being permitted to register the participants to participate in the ANDREWS HOCKEY GROWTH PROGRAMS East Coast Ice Spring Hockey, I, the undersigned parent/guardian hereby release and discharge ANDREWS HOCKEY GROWTH PROGRAMS from any all claims, demands, actions and causes of action by me which I may have for and damages, loss or injury suffered by my child or incurred by me and resulting directly or indirectly from the participation of such child in the program. I hereby undertake to indemnify AHGP and its servants, agents and employees and hold them harmless from and in respect of any and all claims, demands, actions and proceedings which may be brought by or on behalf of my said child against ANDREWS HOCKEY GROWTH PROGRAMS arising out of his participation in the aforesaid program and in respect of any damages, loss or injury incurred by him during or as a result of such participation, including all costs and expenses incurred in defending any and all claims, demands, actions and proceedings.

Fall Programs:

Payment Terms: HST is not included in the price and will be applied to all camps. For all Fall Programs, payment is non-refundable after August 1st 2016. Note: If you elect to pay with cheque, money order, or cash the spot will be held for two weeks until deposit or full payment is received.

Payment Terms: HST is not included in the price and will be applied to all camps. For all Winter Programs, payment is non-refundable.

Participant Waiver: In consideration of the participant and his/her parent being permitted to register the participants to participate in the ANDREWS HOCKEY GROWTH PROGRAMS I, the undersigned parent/guardian hereby release and discharge ANDREWS HOCKEY GROWTH PROGRAMS from any and all claims, demands, actions and causes of action which I may have for any damages, loss or injury suffered by my child or incurred by me and resulting directly or indirectly from the participation of such child in such program. I hereby undertake to indemnify ANDREWS HOCKEY GROWTH PROGRAMS and its servants, agents and employees and hold them harmless from and in respect of any and all claims, demands, actions and proceedings which may be brought by or on behalf of my said child against ANDREWS HOCKEY GROWTH PROGRAMS arising out of his participation in the aforesaid program and in respect of any damages, loss or injury incurred by him during or as a result of any damages, loss or injury incurred by him suing or as a result of such participation including all costs and expenses incurred in defending any and all claims, demands, actions and proceedings.

Synthetic Ice Programs:

Payment Terms: HST is not included in the price and will be applied to all camps. For all Synthetic Ice Programs, payment is due prior to the first scheduled session. All Sales are final. Packages expire 1 year from the date of purchase. Cancellations must be communicated 24 hours in advance.

Participant Waiver: In consideration of the participant and his/her parent being permitted to register the participants to participate in the ANDREWS HOCKEY GROWTH PROGRAMS I, the undersigned parent/guardian hereby release and discharge ANDREWS HOCKEY GROWTH PROGRAMS from any and all claims, demands, actions and causes of action which I may have for any damages, loss or injury suffered by my child or incurred by me and resulting directly or indirectly from the participation of such child in such program. I hereby undertake to indemnify ANDREWS HOCKEY GROWTH PROGRAMS and its servants, agents and employees and hold them harmless from and in respect of any and all claims, demands, actions and proceedings which may be brought by or on behalf of my said child against ANDREWS HOCKEY GROWTH PROGRAMS arising out of his participation in the aforesaid program and in respect of any damages, loss or injury incurred by him during or as a result of any damages, loss or injury incurred by him suing or as a result of such participation including all costs and expenses incurred in defending any and all claims, demands, actions and proceedings.
